Please print this page and save for your records. This confirms that you have REGISTERED for the conference. This does NOT confirm payment. Payment or Purchase Order in the amount shown above must be received in our office within 15 days of submitting your registration.

Registration deadline is February 6, 2012.  Registrations are limited – First come, first served.

 Cancellations:  A $15.00 administrative fee will be charged for any cancellations. To request a refund before February 6, 2012, send a letter to the Conference Manager at the address listed below. No refunds will be issued after February 7, 2012.
